Step 1 - Retainer Agreement. Read, Sign & Submit (Click to begin)

I, the undersigned, hereby retain HOOPIS & HOOPIS to represent me in my Domestic Relations’ case. I agree to pay the sum of One Thousand Five Hundred Dollars ($1,500) as an immediate flat rate retainer or services through the Rhode Island Family Court mediation process. Thereafter, I agree to pay the sum of Three Hundred Dollars ($300.00) per hour with the understanding that the retainer paid will be applied towards the total fee charged. I acknowledge and agree that other attorneys with this office may handle different aspects of my case including Court appearances, but that Jennifer Hoopis D’Ambra, Esq. shall coordinate and supervise all actions taken on my behalf. I also hereby agree that in addition to the above payments, you may retain for yourself such sums of money as your Plaintiff/Defendant shall be called upon by the Court to pay as counsel fees as a result of any necessary court proceedings. In the event of a negotiated settlement, I understand that you will attempt to secure counsel fees on my behalf from your Plaintiff/Defendant in order to reduce the amount of counsel fees which I may be obligated to pay to you. It may be necessary to hire certain experts, such as accountants, real estate appraisers, and other experts in order to properly prepare and present my case. No experts will be retained without my express approval. I understand that all expenses for such experts shall be my sole obligation and responsibility and such expenses are not included in my attorney's fees. Any costs incurred or expended will be billed separately. All fees shall be paid in full prior to the entry of Orders

Step 4 - Birth Certificates (click to begin)

Please forward a certified copy of your child’s birth certificate; if you do not have one in your possession, copies can be obtained for a fee from the City/Town clerks where you lived when your child was born or the Rhode Island Department of Health. (You can use the Upload Documents Tool on this page to send to the attorney)
